HEARING

• No formal income guidelines
• Completion of Financial Application Form
• Explanation of Insurance amount
  • Request of monetary amount
• Payment must be Medicaid pricing

SPEECH THERAPY

• NO INSURANCE COVERAGE
  • Child must receive speech/language therapy in the school
  • Completion of Financial Application Form
  • Explanation of Insurance Benefits
  • Referral to Scottish Rite for determine of services
  • Payment must be Medicaid pricing
  • Maximum donation: $1,000.00

• MEDICAL INSURANCE COVERAGE
  • Child must complete a speech/language evaluation and provide recommendations
  • Completion of Financial Application Form
  • Explanation of insurance benefits
    • submission of Explaination of Benefits (EOB) one time per month to ensure appropriate costs.
  • Maximum donation: $500.00
